When calling to schedule a new patient appointment, please have the following information available:
- Name, address, and phone number of patient
- Reason for your visit
- Your insurance information (insurance name, name of medical group if applicable, ID number)
- Referral/ authorization number, if applicable
- Name of referring doctor
If your insurance information or coverage changes before you come in, please notify us!
