Billing and Insurance

We accept most major medical insurances and PPOs. We are also contracted with several HMO plans, including the following:

Patients that require referrals from their medical group are responsible to obtain authorization from their PCP for first-time visits and when returning after being dismissed from follow-up. Patients that wish to be seen without a valid referral will be held responsible for any charges rendered on the given date of service.

It is the patient’s responsibility to ensure that they are eligible with their provided insurance and, when applicable, with their provided HMO. We will do our best to verify that patients remain eligible but cannot bill the insurance when changes have been made without notification prior to service.

Statements are sent out on a monthly basis to any patient with balances left by their insurance. If you have any questions or issues with your balance, call us at 626-918-1881 x115. Please provide us with your account number listed in the upper right portion of the statement. The balance is your responsibility even if you change your address and phone number; please notify us if you are in the middle of changing either.
